Therapists and their healthcare provider entities in all settings have difficulty with manual documentation. It is time consuming, tedious and repetitious, often requiring extended comments or very specific use of semantics. Documentation after the initial evaluation demands extensive transcription of information collected during prior events. All of this contributes to inefficiency, inconsistency, and an increased likelihood of human error.

The negative aspects of manual documentation are widely recognized. In 2001, the Institute of Medicines Committee on the Quality of Health Care in America released its report Crossing the Quality Chasm: A New Health System for the 21st Century. It called for the elimination of most handwritten clinical data by the end of the decade, through the development of an information infrastructure to support healthcare delivery. The IOM study on quality repeatedly refers to the value of information systems to reduce manual transcription and other sources of human error and to increase efficiency.
